Leg Swelling and Vein Disease
Swelling that builds through the day and settles overnight is a pattern worth investigating — it is one of the more common ways venous disease presents.
A Clinical Overview
Swelling in the lower leg and ankle — clinically, peripheral oedema — happens when fluid leaves the circulation and collects in the surrounding tissue. There are many reasons that can occur, and not all of them involve the veins. Heart, kidney, liver, thyroid and lymphatic conditions all cause swelling, as do several common medications.
Venous swelling has a characteristic rhythm. Leg veins carry blood upward against gravity, helped by one-way valves and the pump action of the calf muscle. When those valves fail, blood pools and pressure in the leg rises — a condition called chronic venous insufficiency. That raised pressure pushes fluid out into the tissue. Because it is driven by gravity, the swelling accumulates over a day of standing or sitting and reduces overnight when the legs are level.
That daily pattern is a useful clue, but it is not proof. The purpose of an evaluation is to determine whether your veins are the explanation — and to say so plainly if they are not.
Who May Benefit From an Evaluation
Consider a vein evaluation if you notice:
- Ankle or lower-leg swelling that is mild in the morning and worse by evening.
- Swelling alongside aching, heaviness or fatigue in the same leg.
- Sock or shoe marks that stay pressed into the skin.
- Swelling together with visible varicose veins or spider veins.
- Skin changes near the ankle — darkening, thickening, itching or dryness.
- Swelling that has persisted for weeks without a clear explanation.
How Venous Swelling Is Assessed
The examination that answers the question is a duplex ultrasound. At Avancé Vein Care this scan is performed personally by Dr. Tadepalli — the physician who reads the study is the physician who acquired it.
The scan is done standing, because reflux is a gravity-dependent finding and can be missed lying down. It maps which vein segments leak, in which direction and for how long, and checks for clot or obstruction. That map is what determines whether treatment is appropriate and, if so, which approach suits your anatomy.
What Treatment May Involve
If the ultrasound shows no venous cause, you will be told that directly and pointed toward the appropriate evaluation. If it does show reflux, management usually proceeds in steps:
Conservative measures first
Graduated compression, leg elevation, calf-muscle activity and weight management all reduce venous pressure. Most insurers also require a documented trial of conservative therapy before authorising a procedure, so this stage serves a practical purpose as well as a clinical one.
Treating the underlying reflux
When a specific vein is shown to be the source, closing it redirects blood into healthy veins. Depending on the vein's size, depth and course, that may mean radiofrequency ablation, endovenous laser ablation, Varithena® or VenaSeal™. These are office-based procedures performed under local anaesthesia.
What to Expect Afterwards
Swelling generally responds more gradually than aching does. Many patients notice heaviness easing within weeks, while swelling improves over a longer period and often continues to require compression — particularly if the swelling is long-standing or has more than one cause. Where tissue changes have been present for years, the realistic goal may be control and prevention of progression rather than a return to a completely normal-looking leg.
Limitations and Honest Expectations
Treating venous reflux addresses the venous contribution to swelling and nothing else. If heart, kidney, lymphatic or medication factors are also involved, they need managing in their own right, and swelling may persist despite a technically successful vein procedure. Where lymphatic drainage is impaired as well as venous return, the picture is a mixed one and is managed differently.
